Our Mission

To promote health and wellness among  the Cheyenne-Arapaho community, to educate tribal members in the prevention of  diabetes, empower our diabetic patients to cope with the complications of diabetes and provide support for healthy lifestyle choices.

Target Population 

Enrolled Cheyenne-Arapaho Tribal Members or members of a Federally recognized Tribe that resides within the 10-county Cheyenne-Arapaho service area.

COMMUNITY FITNESS SERVICES

The Fitness Staff is dedicated to providing support for individuals and groups in their quest for healthier lifestyle. The Fitness Staff works with the Cheyenne–Arapaho Child Care Program, Head Start Program and local schools to provide additional support for fitness goals of these programs.

The Registered Nurse provides:

¨ CASE MANAGEMENT

¨ BLOOD PRESSURE MONITORS

¨ PROVIDES DIABETIC SUPPLIES

¨ DIABETES EDUCATION

¨  DIABETES WELLNESS EYEGLASS PROGRAM - Offers  voucher  towards the    purchase of eyeglasses for  Diabetic  patients who meet the program  requirements.

¨  Assistance  may only be utilized once per year.

¨ We are not responsible for lost, broken, or stolen glasses.

¨ Over 150 quality frames available to choose from, as well as, various lenses, tints, and offer bi-focal, or tri-focal options.

¨ New expanded payment arrangements with Cheyenne-Arapaho Tribal Elder Care Program, Crisis Assistance Program, and the Johnson O’Malley Program. See each program for further details.

¨  Services are also available for private pay,   public and tribal employees.

¨  Pay roll deduction option available for tribal employees.

The Diabetes and Wellness Program offered by the Cheyenne-Arapaho Tribes is intended to empower our tribal members giving them the tools necessary to make healthy lifestyle choices. It is aimed at preventing diabetes and enabling our diabetic patients to cope with the complications of this illness.
